How to Apply for MHRT/C Certification

Second time applicants, scroll down to near the bottom of this page.

For first time applicants:
Please follow all FOUR steps.

Step 1: Determine your Pathway. Each Pathway requires different documents to apply.

Step 2: Gather the required documents depending on your Pathway as follows:

Pathway A: An official transcript. It can be original or a legible scan/photo of the official transcript. Our office also accepts an electronic transcript emailed directly from the institution. Email: cfl@maine.edu.

Pathway B: The completion certificate from Maine’s Mental Health System 101
AND an official transcript. It can be original or a legible scan/photo of the official transcript. Our office also accepts an electronic transcript emailed directly from the institution. Email: cfl@maine.edu.

Pathway C: The completion certificate from Maine’s Mental Health System 101
AND EITHER
An official transcript. It can be original or a legible scan/photo of the official transcript. Our office also accepts an electronic transcript emailed directly from the institution. Email: cfl@maine.edu.
OR
A completed Work Verification Form
PROCEED TO STEP 3

Step 3: Complete an application form and submit it with your documentation to cfl@maine.edu
You can download a PDF form, a Word form, or use our online web portal. If you opt for the web portal, be sure you have all your documentation ready prior to logging into the web portal. There is a limit of six attachments on the web portal.

Step 4: You will receive a confirmation of receipt email within 2 business days. It will indicate if your paperwork is complete or if you are missing any documents. Please forward any missing documents in a reply to that email.

If complete, please allow our staff up to 3 weeks to review your application. Staff reviews complete applications in the order received.

Other optional documents you may need/want to submit:

Name change documentation. If your name is different on your transcript, you must provide documentation of your name change. We accept a legible scan or photo of whichever of the following applies:

Marriage License
Divorce Decree—page one showing your previous name and whichever page shows your new name
Court Legal Name Change Petition

Non-Academic Training certificates

Completed Credit for Work Experience forms with the supplementary narrative from your clinical supervisor.

Documents we do not need and will not review:

MHRT I-related trainings (CPR, First Aid, MHSS, behavioral intervention)
Children’s Services trainings (Example: BHP)
Birth certificate
Driver’s license
Resume
General letters of recommendation

REMINDER: Staff cannot review applications until they are complete with all necessary documentation.

Second time Applicants for your second certificate:

Complete another application form or log back into the web portal (see step 3 above). For your second Pathway B or C certificate, check the fifth box on the application form. If you have completed all remaining Domains, check the first box. Submit your documentation with the application. Refer to Step 4 above.

If you are using the web portal, after updating your contact information, DELETE ALL PREVIOUS ATTACHMENTS before uploading new ones. Please remember the web portal is limited to six attachments.
